I'm starting this thread to try to collect the sources of the major and minor versions of each buff.
For those who may not know, in 1.6 many skills have been modified to give certain buffs, and these buffs are not stackable. Each buff has a major and minor version. You can have a major and a minor version of the buff active at the same time, but you cannot have multiple major versions of the buff or multiple minor versions of the buff active at the same time.
This list should be helpful for theory-crafting (a lot of builds will need rethinking) and to make sure you're not duplicating a buff and thus wasting a skill.
Berserk - Increases All Damage Done
